Ghrelin is a potent stimulator of growth hormone secretion from the anterior pituitary gland. The ghrelin receptor is a G protein-coupled receptor, known as the growth hormone secretagogue receptor. Ghrelin binds to the GHSR1a splice-variant of this receptor which is present in high density in the hypothalamus, pituitary as well as vagal afferent cell bodies and vagal afferent endings throughout the gastro-intestinal tract.

Essentially a synthetic version of ghrelin analogue, GHRP-6 (like GHRP-2) stimulates the release of an endogenous growth hormone (GH) within the somatotropes of the anterior pituitary in the animal and human body. Specifically, GHRP-6 will increase the number of somatotropes in a GH pulse by limiting the amount of somatostatin present, while standard GHRH increases the amplitude at which the pituitary cells pulse. Unlike ghrelin, GHRP-6 is not specifically used to increase appetite, but it may have secondary actions that impact hypothalamic neurons. These effects last for approximately an hour after the initial application, which mimics the natural application of GH, and consists of an eight hour circulation period.
Serum ghrelin levels vary as a function of energy balance. Ghrelin levels are increased in anorexia and decreased in obesity.78 Thus, it is possible that ghrelin may be an important player in food intake behavior and perhaps in chronic over- and under-nutrition as well.9 Because of its dual effects, ghrelin may be a critical hormonal signal of nutritional status to the somatotropic axis, playing a role in integrating energy balance with the growth process.10

The experiment in rats, based on clean full-thickness controlled wounds, indicated that GHRP-6 pharmacodynamics has likely involved attenuation of immunoinflammatory mediators, their effector cells, and the reduction of fibrosis-inducing cytokines. The concerted action of these two elemental mechanisms may have theoretically translated into a particular modulation of fibroblasts response to injury, leading to precocious closure with a reduced scarring. Outstandingly, the mechanisms underlying this pattern of healing do not appear to interfere with the angiogenic repopulation nor with the reepithelialization process.

Five public submissions were received. Many of the submissions referred to the article published in the New England Journal of Medicine (NEJM) when giving their reasons for either supporting or rejecting the proposal. Some submissions also noted that a similar proposal is to be considered by an upcoming meeting of the Medicines Classification Committee (MCC) in New Zealand.
During the last 15 years, a plethora of experimental evidence supports the pharmacological benefits of the exogenous administration of synthetic growth hormone-releasing peptides (GHRPs). In parallel to their growth hormone-releasing action, these agents exert cytoprotective effects encompassing cardiac and extracardiac organs [8]. GHRP-6 is a class of peptidyl GH secretagogue, similar to met-enkephalin, that has reproducibly shown antinecrogenic and antiapoptotic properties in multiple experimental scenarios, including ischemia/reperfusion [9–11]. Globally speaking, exogenously administered GHRP-6 has broadly been shown to act as a prosurvival factor for cells and tissues threatened by otherwise lethal insults.
For example, there are Growth Hormone Releasing Peptides (GHRP’s with names such as ipamorelin and hexarein) which allow for a slow and steady growth hormone release that produces a pulse which mimics natural growth hormone release times and Growth Hormone Releasing Hormones (GHRH’s such as Mod-GRF) for an even stronger natural release of growth hormone and greater presence of growth hormone precursors known as GH “frags” or fragments.

The interim decision was to include in Schedule 4 and in Appendix D Item 5 Growth Hormone Releasing Hormones (GHRHs), Growth Hormone Secretagogues (GHSs), Growth Hormone Releasing Peptides (GHRPs) as well as new individual substance entries for CJC-1295, ipamorelin, pralmorelin (Growth Hormone Releasing Peptide-2), Growth Hormone Releasing Peptide-6, hexarelin and AOD-9604.
Finally, an exciting medical opportunity could be opened for synthetic GHRP to treat the threatening cancer-associated anorexia–cachexia syndrome in advanced-stage cancer patients. Although the mechanistic bases of this syndrome are not fully understood, it represents a major impediment for the course of chemotherapy. In a rodent model of cancer-bearing chemotherapy, GHRP-2 administration increased appetite/food intake and prolonged median survival time, which certainly suggests that GHRP-2 may improve the quality of life of cancer patients by correcting its nutritional and metabolic states.61 These data may also incite to further studies in the search for a potential niche for GHRP to counteract the catabolic states of prolonged critical illness, invasive surgeries, severe burn traumas, etc.
A seminal report by a Merck Research Laboratories group dated 2003 demonstrated for the first time that chronic treatment with GHRP-6 (21 days) prevented sudden death in a canine model of DCM and subsequently subjected to acute myocardial infarction (AMI). In the meantime, the mortality rates for the vehicle and GH-treated groups were about 50%. Although the authors do not precise the mechanism underlying the 100% survival in the GHRP-6 group, an enhanced regional myocardial compensatory function of the nonischemic zone was assumed.40 This notion could be validated at least in part by the fact that the cardiotropic effects shown by GHRP-1, GHRP-2, GHRP-6, and hexarelin in cardiomyocytes and isolated, denervated, perfused hearts are mediated by an elevation of Ca2+ influx through the voltage-gated calcium channel, triggering Ca2+ release from thapsigargin-sensitive intracellular stores, which translated in a positive inotropic response without a chronotropic effect.41 More recent data confirm the ability of hexarelin and other secretagogue peptides that bind and activate the GHS-R1a, to control the cardiac action potential and reduce apoptosis of cardiomyocytes, derived from isolated hearts subjected to ischemia/reperfusion episodes.42

You’re no doubt taking it for the fairy tale positive side effects, which have already been outlined, but like any caper about something enchanted, the magic comes with a price. For GHRP6 these can include flu-like symptoms, joint aches, headaches and water retention.
