Immunohistochemical determination of CD31 expression (platelet endothelial cell adhesion molecule-1, PECAM-1) was conducted as this is a marker protein of mature vascular endothelium . Sections (5 μm) were mounted on chromalum-coated slides, dewaxed, rehydrated, rinsed, and washed in PBS 1x solution for 30 min. Once endogenous peroxidase was quenched, the specimens were treated with target retrieval solution (Dako) equilibrated at 99°C. Tissue samples were then incubated for 40 min with 1/50 dilution of anti-CD31 antibody (Abcam 28364, USA) in background reducing solution (Dako). The immunohistochemical reactions were carried out using the labelled streptavidin/biotin-horseradish peroxidase conjugate method, according to the manufacturer’s instructions (Dako). The peroxidase reaction was developed with diaminobenzidine and counterstained with hematoxylin.

From the standpoint of protein synthesis and muscle repair, IGF-1 injections have also been shown to enhance the anticatabolic effects of insulin and to increase the protein synthesis normally induced by growth hormone. This is because, like insulin, IGF-1 encourages amino acid uptake into muscle cells, stimulates peripheral tissue uptake of glucose (which lowers blood glucose levels), and suppresses liver glucose production. That last fact is important and is actually why IGF-1 is even being considered as a diabetes-prevention drug. For the first time, the acute administration of hexarelin proved to induce a positive inotropic effect in humans, which is GH independent and mediated by specific myocardial receptors for a GH secretagogue peptide.29 A subsequent study involving hexarelin administration to normal adults, severe GH-deficient patients (N = 7), and patients with severe ischemic DCM (N = 12) confirmed that the acute administration of hexarelin exerts a GH-independent positive inotropic effect likely mediated by specific GHRP myocardial receptors.30 This pioneering group subsequently evaluated the cardiac performances of the acute hexarelin administration (2.0 µg/kg, i.v.) in patients undergoing bypass surgery in comparison to patients given GH-releasing hormone, recombinant human GH, or placebo. The study concluded that the acute administration of hexarelin improved cardiac performance without any relevant variation in systemic vascular resistance and induced a reduction of wedge pressure and, significantly, that these cardiotropic effects were not shown by the other concurrent interventions.31

For increasing GH levels, GHRP-6 is less effective in the presence of high blood glucose levels or high somatostatin levels, which result from high IGF-1 levels. For this reason, for best effect GHRP-6 should be taken while blood sugar is relatively low, for example about 30-60 minutes before a meal. GHRP-6 will have reduced effect if GH is being taken by injection, because GH increases IGF-1. Where GH use is limited to no more than about 14 IU per week, simultaneous GHRP-6 use probably still will increase GH somewhat further, but if GH use is greater than this then likely GHRP-6 injections will do little to nothing towards increasing GH levels any further.
Since GHRP-6 influences natural GH release, rules for its administration are similar to those when administering GH itself, i.e. do not have the dose within a 2 hour window after a meal, and/or wait 30 minutes till eating after dosing, due to any endogenous insulin release from the meal possibly stunting the release of natural GH and its uptake by the relevant cells.

Similar to the effects of ghrelin in male subjects GH, ACTH, and cortisol are elevated. In a set of studies, the intranasal, oral, and sublingual administration of GHRP-6 was tested. By these routes of administration the effects of the peptide on the sleep EEG and on hormone secretion are less distinct and partly different from those after intravenous injection. In contrast to the sleep-promoting effects of GHRP-6 and ghrelin, hexarelin prompts a decrease of SWS, whereas the pattern of endocrine effects after hexarelin resemble the endocrine changes after ghrelin and GHRP-6 in that there is a marked stimulation of GH. The decrease of SWS after hexarelin may be related to negative feedback inhibition of endogenous GHRH. After a single dose of GHRP-2 during the third period of REM sleep, sleep remains unchanged. The lack of effects in this study may be related to the method that uses only a single injection of the substance.
