Tβ4 has been used at our clinic with a great deal of success in patients suffering from Lyme disease or other infectious and/or inflammatory conditions. Tβ4 is very well tolerated and has not been found to cause any significant side effects. There is no “one right way”, to use Ipamorelin. For example, if you are using 500 to 1000 mcg doses daily, twice a day, your cycle might run for an 8 week period. All these events could be primarily presided by the GHRP-6-related reduction of TGFB1 and CTGF expression in both parenchymal and nonparenchymal cells [7].

Both ex vivo and in vivo, GHRPs offered a striking heart protection against reperfusion stunning, improved ventricular pressures and volumes, and reduced CK concentration in perfusate. Again, they sustained the concept that the protection afforded by the peptide is likely due to a direct cardiotropic action that appeared far greater than that induced by GH administration in a concurrent control group.27 A more defining protocol was assumed in 1999 as the study included hypophysectomized rats, to ascertain whether hexarelin had non-GH-mediated protective effects on the heart. The authors showed that hexarelin attenuated the ischemia/reperfusion damage and prevented elevation of LV end-diastolic pressure, coronary perfusion pressure, reactivity of the coronary vasculature to angiotensin II, and the release of creatine kinase in hypophysectomized animals.28 These three experiments were pivotal to define GHRP intrinsic cardioprotective ability.

The potent biologic effects of GHRPs and the identification of the GHS-R suggested the existence of a natural ligand for the receptor that is involved in the physiologic regulation of GH secretion. The acylated peptide ghrelin, produced and secreted into the circulation from the stomach, is this ligand (Fig. 7-22). The effects of ghrelin on GH secretion in humans are identical to or more potent than those of the non-natural GHRPs (see Fig. 7-20). In addition, ghrelin acutely increases circulating PRL, ACTH, cortisol, and aldosterone levels. There is debate con­cerning the extent and localization of ghrelin expression in the brain that must be resolved before the implications of gastric-derived ghrelin in the regulation of pituitary hormone secretion are fully understood. Furthermore, post-translational processing of pro-ghrelin gives rise to a second neuropeptide, obestatin, which may also have functional roles in activity of the GH/IGF-1 axis and metabolism. A proposed role for pro-ghrelin peptides in appetite and the regulation of food intake is discussed in Chapter 35.

During the last 15 years, a plethora of experimental evidence supports the pharmacological benefits of the exogenous administration of synthetic growth hormone-releasing peptides (GHRPs). Where GH use is limited to no more than about 14 IU per week, simultaneous GHRP-6 use probably still will increase GH somewhat further, but if GH use is greater than this then likely GHRP-6 injections will do little to nothing towards increasing GH levels any further.
Following the preliminary histological data, suggesting a reduction of wound inflammation and a far more organized ECM, we addressed the gene expression study toward inflammatory and profibrogenic markers.
